Register with the Practice

If you would like to register with the practice you can come into the surgery and complete one of our registration forms and new patient questionnaires, alternatively you can complete the forms via the link below which directs you to the NHS App. Please note sometimes it can take up to 10 working days from us receiving your completed forms to us registering you on our clinical system so if you are taking regular prescribed medication you will need to tell us what medication you are taking. If you do not have more than 14 days supply of your medication please ensure you order them from your current GP practice who can arrange to send your prescription electronically to a local pharmacy of your choice. One of our doctors or practice pharmacists will then review your list of medication which may result in you being asked to make an appointment.

To speed up your registration please bring in proof of your address such as a utility bill or driving licence, alternatively you can email a photograph to :

In order for us to complete your registration we do require proof of address.  If you are already a patient with us and would like to have online access to your notes or our online booking system we may ask you for additional ID.

From July 2024 the practice now records all incoming and outgoing telephone calls.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Patient Information – Choice of GP Practice

At Whittington Moor Surgery patients are allowed to register if living outside the practice boundary. All GP practices in England are free to register new patients who live outside their practice boundary area.

This means that you are able to join practices in more convenient locations, such as a practice near your work, closer to your child’s school or even re-join your existing practice when you move home.

There are some important things you need to know before choosing to register with a GP practice away from your home address. Things such as:

  • These new arrangements are voluntary for GP practices. If the practice has no capacity at the time or feels it is not clinically appropriate or practical for you to be registered so far away from home then they can still refuse registration. The practice should explain their reason to you.
  • Once you have decided to register with a GP practice away from your home address, you will need to choose a practice that is accepting registrations from outside of their traditional practice boundaries. You can find this information by contacting the GP practice directly. They may have this information on their practice website or on their practice leaflet but the best way will be to contact them by telephone and asking a receptionist.
  • Because of the greater distance to your home, the GP practice may register you without the responsibility to provide you with a home visit when you are not well enough to go to the practice yourself.
  • When you register with a practice away from home, without home visits, you will be given information about what you should do in those circumstances where you need to access care whilst at home.
  • You may, if your circumstances change and it is no longer clinically appropriate or practical for you to be registered away from home, be asked to consider registering with a practice closer to home where your care may be better provided at that time.
  • Should you become too ill to attend your registered practice in person or your practice advises that they are unable to assist you over the phone, contact NHS111 who will direct you to the right service for your needs.
